2009 Band Camp Information 

This years Band Camp is July 12th thru July 17th at Rio Grande. The busses and Directors will be at the High School after 11:00 a.m. ready to start loading luggage.  The busses are scheduled to leave at 1:00 p.m. All students should plan on arriving at the High School between 11:00 and Noon. Any help from parents with loading luggage is always welcome.

Click here for your COMPLETE informational packet and mandatory forms needed for the 2009-2010 Band Camp and Marching Band season.  or you can pick just the form you need below.  Sorry that there is some duplication of information between forms but this is unavoidable since Rio Grande College requires their own form completed as well as the Tipp City school system.  Thank you for your patience in completing all required forms.

#1. Rio Grande Emergency Medical Release Authorization (Required)

Required by the college & maintained by their medical staff

 

 #2. THS Music Department Health Information (Required) (Orange)

This form is for the high school’s use at band camp or any other band related trips.

Requires an attached copy of your insurance card information

 #3. THS Band Medication Form for “OVER THE COUNTER” Medications (Required)  (Green) This does NOT need a physician’s signature – but DOES need a parent signature and is required for all students. 

#4. TIPP CITY SCHOOL DISTRICT PRESCRIPTION MEDICATION FORM Parent Side (Page 1) (Yellow)

    Physician Request for MEDICATION FORM Physician Side (Page 2) (Optional) 

This is the school systems “YELLOW” form that they use for all medications PRESCRIBED by a physician & it DOES REQUIRE A PHYSICIAN SIGNATURE.

 

ALL FORMS NEED TO BE COMPLETED & RETURNED TO THE MUSIC DEPARTMENT BY JULY 6, 2009

 This is the date of the MANDATORY BAND CAMP meeting which will be held Monday, July 6, 2009 @ 7:00 p.m. in the Commons at THS.

 Remainder of BAND CAMP MONEY is also due July 6, 2009.
